Schedule a New Patient Consultation:

• Use this link to schedule if you have a chronic, long-term illness, disease, disorder, syndrome, or symptom. During this consultation, we will go over your complete medical history in order to design a comprehensive, long-term treatment plan.
• If you have previously received a custom formula from Estuary Herbs, but it has been more than one year since your last consultation, please use this link to schedule.

Schedule a Returning Patient Consultation:

• Use this link if you have already established care with Hayley Gardner MSOM DCCM LAc or Lan Nguyen MAOM, and it has been less than one year since your last consultation.

Schedule an Herbal Urgent Care Consultation:

• Use this appointment type if you have acute symptoms, illness, or injury that just came up within the last 72 hours.
• Examples: head cold, stomach bug, UTI, migraine, injury, infection, cough, earache.
• This appointment is NOT to be used in cases of true medical emergency. If you have a medical emergency, call 911 or go to the nearest Emergency Room.
• If you are a long-term returning patient seeking a formula adjustment related to your ongoing care, you may use this appointment type ONLY if your practitioner has instructed you to.
